Please use the form below to begin the process for renewing your instructor certification. After selecting your renewal options and completing this registration, one of our Training Center Faculty members will contact you to arrange your course monitoring. Your course monitoring must be scheduled at least 30 days prior to your expiration. The monitoring and renewal paperwork must be completed at least 10 days prior to your expiration. Full details on the program will be included in the instructor renewal packet that will be emailed after you complete this registration. Your instructor renewal will renew BOTH your instructor and Provider certification. You will receive both certification cards upon successful completion. Please note that instructor renewal fees are non-refundable once paid.
